I hereby declare that I have read, understood, and have been duly informed, both verbally and in writing, in a language I understand, of the “Personal Data Processing Disclosure Notice” of UZM. DR. ECEM CANTÜRK NAZLI and all my rights stipulated under the applicable legislation as clearly stated therein.
By accepting the Disclosure Notice, I expressly consent to the PROCESSING of my personal data, including my contact information, which I have agreed to be processed within the scope of the Disclosure Notice, by UZM. DR. ECEM CANTÜRK NAZLI for the purposes of carrying out medical examinations, preventive healthcare, medical diagnosis, treatment, care, and follow-up services; improving the medical treatments and procedures applied to me; reminding appointment dates for ongoing treatments; and informing me directly about developments and innovations related to medical treatments and procedures.
